What is HoLEP Surgery?

HoLEP — Holmium Laser Enucleation of the Prostate — is the most effective minimally invasive surgery available for benign prostatic hyperplasia (BPH), or enlarged prostate. Unlike older procedures that only reduce prostate tissue, HoLEP removes the entire obstructing portion, addressing the root cause of obstruction with unmatched durability.

Warning Signs That Mean It's Time to Call

Many men tolerate urinary symptoms for years before seeking help — often because they assume it's just aging, or because they're not sure whether their symptoms are serious enough. Some symptoms are more than inconvenient. They are warning signs of prostate obstruction that, left untreated, can cause permanent damage to the bladder and kidneys.

Complete inability to urinate
Also called acute urinary retention. If you cannot urinate at all, this is a urological emergency — call the office or go to an emergency room immediately.
Blood in the urine
Pink, red, or brown-tinged urine — even if it resolves on its own — should always be evaluated. It can indicate prostate enlargement, bladder issues, or other conditions that need assessment.
Recurrent urinary tract infections
Two or more UTIs in a year suggests incomplete bladder emptying caused by obstruction — a breeding ground for bacteria. This requires evaluation, not just repeated antibiotics.
Kidney function changes
If your doctor has noted rising creatinine levels or signs of back-pressure on your kidneys, untreated prostate obstruction may be the cause. This can become irreversible without timely treatment.

Waking up 2+ times per night to urinate
Nocturia is one of the most disruptive BPH symptoms and one of the most dramatically improved by HoLEP. Most patients go from 4–5 trips a night to once or none.
Weak, slow, or interrupted urine stream
Straining to urinate, a stream that stops and starts, or a noticeably weak flow are classic signs of progressive obstruction.
Feeling of incomplete bladder emptying
Leaving the bathroom and immediately feeling like you need to go again, or knowing your bladder never fully drains, indicates significant residual urine — a sign of obstruction.
Symptoms not controlled by medication
If you're already taking Flomax, finasteride, or both and your symptoms continue to worsen, medication is no longer managing your condition. It's time to talk about definitive treatment.

A Life Without Daily Medication

Stop Taking Flomax. Forever.

Most men with BPH have been on Flomax (tamsulosin), finasteride, dutasteride, or a combination for years. Their doctors renew the prescription every year. The pills become part of the routine. And most men assume this is just how life is now.

It doesn't have to be.

After a successful HoLEP procedure, the vast majority of patients discontinue all BPH medications immediately — the same day as surgery, or within days. Because the obstructing tissue has been completely removed, the mechanical reason for those medications no longer exists. They are not tapered. They are not gradually reduced. They are simply stopped.

Most patients never need to start them again.

Flomax / Tamsulosin
An alpha-blocker that relaxes the prostate and bladder neck muscles to improve flow. Common side effects include dizziness, retrograde ejaculation, and low blood pressure — especially dangerous when rising from bed or a chair. After HoLEP, there is no obstructed muscle to relax. Discontinued immediately after surgery.
Finasteride / Dutasteride (5-alpha reductase inhibitors)
Hormonal medications that shrink the prostate by blocking testosterone conversion. Take 6+ months to work, can affect libido and sexual function, and must be taken indefinitely — the prostate regrows if stopped. After HoLEP, the adenoma has been removed. No tissue left to shrink. Discontinued immediately.
